    The 2005 NBA All-Star Fantasy Camp is produced under agreement between NBA Entertainment, a division of NBA Properties, Inc., and Crain Sports Entertainment, LLC d/b/a Birdsong Sports & Entertainment. Detailed event information (including complete itinerary) will be sent to you upon receipt of the completed application and payment. In the event of a “sell-out”, priority will be given to the postmark date on the application. Cancellations must be received in writing by January 31, 2005 and will result in a penalty of $750.00. Camp attendees must be over the age of 21.
