1. HERE'S HOW TO ENTER: Handprint your name, age, street address and phone number, e-mail, school and grade, as well as the reason you are a “Wells Fargo Team Captain of the Week”, on an official entry form or a plain piece of 8.5” x 11” paper and drop off your entry form in the designated box at any Sacramento area Wells Fargo locations or mail to: Wells Fargo Youth Team Captain of the Week, c/o Sacramento Kings, One Sports Parkway, Sacramento, CA 95834. Enter as often as you like, but limit one entry per store visit. No photocopied or mechanically reproduced entries. No responsibility is assumed for lost, late, illegible or misdirected entries. All entries must be received by April 1, 2004. If the entry is not picked in the next weeks selection, it will be pushed into the proceeding weeks selection through the end of the contest. By entering, entrant agrees to hold harmless and indemnify Maloof Sports and Entertainment, Wells Fargo Bank, N.A., the National Basketball Association and its Member Teams, NBA Properties, Inc., and their parents, affiliates, distributors, suppliers, subsidiaries, directors, officers, governors and advertising, promotional or judging agencies against any and all liability, damages or causes of action (however named or described), with respect to or arising out of either entrant’s participation or the receipt or use of the prizes awarded in the promotion. Contest begins October 29, 2003 and ends April 1, 2004.
2. PRIZE: The Wells Fargo Youth Team Captain of the Week will receive two tickets to attend a regular season Sacramento Kings game, date to be chosen by Wells Fargo and the Sacramento Kings. Winner will receive a tour of ARCO Arena and participate in a pre-game on-court ceremony as designated by sponsors. Approximate retail value of prize is $200. Meals, gratuities and other expenses not specifically mentioned herein are the winner’s responsibility. Sponsors assume no responsibility for canceled, delayed, suspended or rescheduled events. Sponsor reserves the right to select seating. An adult must accompany minors at all events.
3. SELECTION: Prizewinners will be selected from among all eligible entries received with a minimum of 10 selections total over the course of the contest. Selections will be held four times during the contest period and will be conducted by a Wells Fargo representative, whose decision shall be final. Odds of winning depend upon the number of eligible entries received at each location. All prizes will be awarded. Students will be selected throughout the 2003-2004 season.
4. ELIGIBILITY: Sweepstakes open to legal residents of the U.S. in Northern California only between the ages of 8-18 years of age. Employees of Maloof Sports and Entertainment, the National Basketball Association and its Member Teams, NBA Properties, Inc., Wells Fargo Bank and their affiliates, distributors, suppliers and advertising, promotional or judging agencies and immediate family members of such employees are not eligible to win. No substitution of prize is offered, no transfer of the prize to a third party is permitted, and non-cash prizes may not be redeemed for cash value except at the sole discretion of the Sponsors. No purchase necessary. Void where prohibited or restricted by law.
5. WINNER NOTIFICATION/RELEASES: All winners will be notified by mail and/or phone. Winners may be required to sign and return an Affidavit of Eligibility and Liability/Publicity Release within ten (10) days of notification before prize will be awarded. In the event the affidavit and liability/publicity release is not returned within ten (10) days, prize may be forfeited and an alternate winner selected. Prize may be forfeited if winner is unable to attend event on date specified. Prize may also be forfeited if the notification is mailed to the address listed on entry form and returned as undeliverable mail without a forwarding address. An adult at prize events must accompany all minors. Except where prohibited by law, winners must consent to the use of their name, voice, picture or other likeness for promotional purposes as determined by Wells Fargo Bank, Maloof Sports and Entertainment, and/or the National Basketball Association and its Member Teams, NBA Properties, Inc., and each of their respective parent, subsidiaries and affiliates.
