Dancing Papas Contest Rules

Qualified participants (former tobacco users or those currently quitting) will be entered into a drawing and three will be randomly selected and paired with a Jazz dancer to compete against each other on November 1 (vs. Houston Rockets), December 26 (vs. Dallas Mavericks) or March 8 (vs. Denver Nuggets). Based on crowd participation, a winner will be selected from each game. On April 2 (vs. Minnesota Timberwolves), the three Dancin' Papas will compete against each other to determine the final winner.

ELIGIBILITY:

All residents may submit nominations regardless of age.

Sweepstakes participants open to residents of Utah 18 years of age or older, except employees and immediate families (spouse, parents, siblings, and children) of The TRUTH™, Utah Department of Health, Papa Murphy's, the National Basketball Association and its Member Teams, NBA Properties, Inc., and each of their respective parents, subsidiaries, affiliates, directors, owners, employees and agents.

Contest void where prohibited.

HOW TO ENTER:

To enter, complete information requested online at http://www.utahjazz.com/thetruth. Promotion begins October 1, 2007. Entries must be received by March 3, 2007.

One entry per household.

By entering, entrant agrees to follow the Official Rules and hold harmless and indemnify The TRUTH™, Utah Department of Health, Papa Murphy's, Utah Jazz and/or the National Basketball Association and its Member Teams, NBA Properties, Inc., and each of their respective parents, subsidiaries, affiliates, directors, officers, employees and agents against any and all liability, damages or causes of action (however named or described), with respect to or arising out of either (i) entrant's participation; or (ii) the receipt or use of the prizes awarded in the promotion.

CONDITIONS OF ENTRY:

No purchase necessary. Chances of winning cannot be determined until all entries are received. Contestants will be chosen randomly.

PRIZES:

For final contest held on April 2, 2008: Grand prize winner receives free Papa Murphy's pizza for a year (up to three large one-topping pizzas per month), a video iPod and Jazz swag. Second and third place contestants receive Papa Murphy's pizza for a year and Jazz swag. The winning prize is non-transferable to any other person, including relatives or friends.

For preliminary contests held on November 1, 2007, December 26, 2007 and March 8, 2008: First prize winner receives a free Papa Murphy's large one-topping pizza, Jazz swag and the opportunity to return to the final contest held on April 2, 2008. Second and Third prize winners receive a Papa Murphy's large one-topping pizza and Jazz swag. All prizes are non-transferable to any other person, including relatives or friends.

Winner is solely responsible for all applicable federal, state and local taxes and any expenses associated with the game, unless otherwise indicated. No substitution, cash equivalent or transfer of prize permitted except at the team's option for a prize of comparable or greater value.

Approximate retail value of prize(s) are: Free Papa Murphy's Pizza for a year is estimated to be worth $360; a video iPod is estimated to be worth $250; Jazz swag is estimated to be worth $100; a large one-topping pizza is estimated to be worth $10.

In consideration for being awarded the prize, winner hereby agrees and consents, without further authorization, compensation or remuneration of any kind, to the use of winner's name and/or likeness in any and all advertising, promotions and other publicity conducted by The TRUTH™, Utah Department of Health, Papa Murphy's, Utah Jazz and/or the National Basketball Association and its Member Teams, NBA Properties, Inc., and each of their respective parents, subsidiaries and affiliates, except where prohibited by law.

DRAWING AND NOTIFICATION:

Winners will be chosen randomly. A list of winners can be obtained by writing the Utah Jazz at 301 W. South Temple, Salt Lake City, UT 84101.

Winners will be notified by phone or email (depending on received information) at least 5-10 days before participation is requested. In the event the winner cannot be contacted, another nominee will be randomly selected.
