The All-Star 7-footer imposed his will on both ends of the court. The Spurs outrebounded the Pacers, 62-40, and made 26-of-33 free throws while Indiana only made 9-of-13.
Duncan has shot 64 percent the past three games and has led the Spurs in scoring in their last eight contests. It was his ninth double-double this season as San Antonio improved to 9-2.
Indiana (8-7) was within 73-68 with 10:34 remaining, but San Antonio went on an 18-3 run over the next seven minutes to put the game out of reach.
The loss was the Pacers' third straight on the second night of back-to-back games.
Antonio Daniels scored 11 points and Steve Smith added 10 for San Antonio, which has not lost at Indiana since December 7, 1999. Rookie Tony Parker added nine points as the Spurs remained undefeated with him starting at point guard.
Jalen Rose and Reggie Miller scored 17 points apiece for Indiana, which lost its third straight to San Antonio.
Pacers rookie point guard Jamaal Tinsley, who had his first career triple-double against Washington on Thursday, managed just four points on 1-of-5 shooting with six assists, three rebounds, five turnovers and five fouls.
Indiana cut the deficit to 66-63 on Miller's 3-pointer with 2:11 remaining in the third quarter. But San Antonio scored the last seven points of the quarter, all on free throws, to take a double-digit advantage into the final period.
