To Apply for Funding Support:
Funding Support Eligibility Requirements:
Financial contributions are limited to non-profit, tax-exempt organizations that have a 501(c)(3) Internal Revenue Service designation and are not classified as a private foundation.
Contributions will only be made to qualified organizations that are located in Oregon and/or southwest Washington and which focus support within these communities.
Applications for financial contributions must be submitted in writing and should be as brief as possible to present the necessary facts about the applying organization and the project for which support is being sought.
All requests for support must be submitted by and on behalf of non-profit, tax-exempt organizations that have a 501(c)(3) Internal Revenue Service designation and are not classified as a private foundation. Priority will be given to requests from organizations whose mission reflects a firm commitment to youth in the areas of fitness, health and education.
Proposals must include the following information:
1. The legal names and address of the organization or agency making application.
2. Proof of tax exempt 501(c)(3).
3. The name, title, address, and telephone number of the person responsible for the activity for which funding is requested.
4. A brief description of the organization and its particular qualifications to carry out the activity for which funding is sought.
5. A summary statement describing the project for which the applicant seeks support, why it is needed and why it merits support.
6. Estimated total of funds required for the project and the amount sought from TBI/OAC.
7. Project timeline.
8. Names of all business organizations involved with the project and a description of their involvement.
9. List of the organizations Board of Directors.
10. Current financial statements (audited if available).
Non-eligible Candidates:
Individuals
Private Schools
Political organizations, campaigns, or candidates for public office
Religious organizations
Fraternal organizations
Trips and tours
Seminars
Debt retirement or operational deficits
Organizations which discriminate against individuals on the basis of race, religion, color, sex, national origin, age, sexual orientation, source of income, handicap status, veteran status or familial status
Organizations which receive 20% or more of their budget from United Way
General operating budgets of tax supported institutions
Foundations which are themselves grant making bodies
All organizations requesting assistance in accordance to the criteria provided may receive one donation within the TBI/OAC fiscal year of July 1 through June 30.

In-Kind Support Eligibility Requirements
The Blazers provide fundraising tools and in-kind services to non-profit organizations throughout Oregon and southwest Washington each season. Resources include:
Autographed team memorabilia for fundraising projects
Ticket opportunities for at-risk youth organizations
Memorabilia:
Requests for autographed memorabilia or Blazers merchandise are limited to auction and/or raffle activities organized to raise fund for non-profit, tax-exempt organizations in Oregon and southwest Washington that have a 501(c)(3) Internal Revenue Service designation. It is our policy not to donate items to be used as prizes or give-aways.
Tickets:
On occasion throughout the season, the Blazers have the opportunity to host youth organizations at games. Priority is given to organizations that focus their efforts on improving the lives of disadvantaged children. Organizations must be non-profit, tax-exempt and located in Oregon and southwest Washington with a valid 501(c)(3) Internal Revenue Service designation.
Proposals must include the following information:
Written requests for non-financial donations must include a description of the planned fundraising activity and must be submitted a minimum of four weeks in advance of event date or beginning of the specified fundraising campaign.
Other in-kind service requests must also be submitted in writing to include information about the organization, number of people serviced, list of board members and executive staff.
Except in limited circumstances, due to the high volume of requests received, donations are limited to one item per organization per year, July 1 through June 30. Organizations receiving a non-financial donation are asked to submit a follow-up document to the Blazers specifying the dollar amount raised from the donated product.
